You can download a copy of any of the documentation used in the Born in Bradford study and other nested studies.  The lists are divided into two sections – general study documentation and questionnaires.  All are available as PDFs.

Questionnaires

• Questionnaires are grouped by the study they are linked to.

• Mothers’ baseline questionnaires are completed by the mother in Bradford Royal Infirmary maternity unit.

• Partners’ questionnaires are completed by the main partner of the carer about themselves.

• Carers’ questionnaires are usually completed by the mother, but this is dependent on family circumstances.
